CONDITIONS OF ATTENDANCE

Access to Venue	:	All access must be through Reception. Please contact the relevant administrative assistant if you have any access/mobility issues.
Special Requirements	:	Please contact the relevant administrative assistant regarding specialised equipment.
Payment	:	Please do not send any money. Schools/Organisations will be invoiced by the Catholic Education Office.
Late Notification	:	If you will not be able to arrive on time, please advise the Administrative Assistant before the course.
Early Departure	:	If you need to leave before the end of a seminar, please speak to the course presenter upon arrival.
Authorisation	:	Where applicable, it is your responsibility to make any necessary arrangement with your Principal or Assistant/Deputy Principal.
Dress	:	Neat professional dress is a requirement at all professional development seminars organised by the Catholic Education Office.
Privacy Statement	:	The primary purpose of collecting the personal information you supply on this form is to process your registration. The Catholic Education Office will not disclose your information to a third party.
Cancellation Notice	:	Unless one week's notice is given for CEWA courses, or two weeks for courses by an external provider, schools or individuals will be invoiced for participants who do not attend workshops/courses for which they are registered.
